Membership Agreement
These Terms and Conditions, together with the Membership Agreement (if applicable), are the entire agreement between you and us relating to the subject matter herein. If you are a member to one of our online membership offerings or products, in the event of any conflict between these Terms and Conditions and the Membership Agreement, the Membership Agreement shall control.

User-Generated Content

Any dispute between you and Health Centers shall be subject to binding arbitration in the state of Utah. You may only resolve disputes with us on an individual basis and may not bring a claim as a plaintiff or a class member in a class, consolidated, or representative action.
Governing Law
These Terms of Service are governed by and construed in accordance with the laws of Utah.
